Observation/Shadowing Orientation Requests

Students who wish to observe/shadow a healthcare professional should follow the steps listed below.  Observation/Shadowing experiences must be less than 40 hours total.

  1. Complete Observation/Shadowing Request Form, and obtain required signatures, from the department/physician that you will be observing.
  2. After receiving required signatures, review the Observation/Shadowing Orientation Module on the St. Luke’s Internet site.
  3. Print and sign Observation-Shadowing Attestation Form. Note:  If under age 18, a parent/guardian will sign the form, in the designated area, as well. 
  4. Once Steps 1 through 3 are complete, return Observation/Shadowing Request Form and Observation-Shadowing Attestation Form to Patient Care Services (Clinical), Human Resources (Non-Clinical), or Medical Staff Services (Physician). Contact information is listed on the Observation/Shadowing Request Form.

All paperwork must be submitted a minimum of three (3) business days prior to date of experience. Late requests will not be accepted.  You may pick up your ID badge at the information desk, inside the Visitor’s Entrance (Entrance 2 for parking), on the day of your observation/shadowing experience.  Please return badge upon leaving the facility.
